Overview

Job Summary & Scope of Responsibilities:  Will be responsible for the oversight of the Kinney Home Services program to ensure patients recently admitted to the hospital are visited and offered services that can be provided by KPH upon date of discharge.

 

This is not a remote opportunity and will be based in Syracuse or Gouverneur, NY.

Responsibilities

Job Responsibilities:

 

  • Responsible for daily communication, coordination, and oversight of the KPH Service Liaison(s).
  • Communicate with Kinney Home Service Liaison regarding patient roster status and estimated and/or confirmed dates of discharge.
  • Assist with the monitoring and coordination of the delivery of health care services for individuals. 
  • Responsible for planning and implementation of services to drive utilization of the Kinney Home Services program.
  • Proactively identify opportunities for increased efficiencies as well as potential barriers to Kinney Home Services program
  • Ability to learn and understand all aspects of the Kinney Home Services program.
  • Responsible for communication to hospital unit managers, discharge planners, physicians, KPH entities as well as external and internal vendors and hospital leadership regarding the progress of the program.
  • Attends discharge meetings as necessary to review areas of concern and create plans for resolution.
  • Responsible for the program budget and reporting directly to the CFO of the program and the KPH Leadership team.
  • Responsible for the creation, tracking, and corrective action plans of KPI’s required for the quality and success of the program.
  • Strong focus on quality to understand and embraces the importance of our Quality Assurance Policy & Procedures; understands and embraces all patient care programs and initiatives; understands and embraces all policies and procedures; accuracy with cash and related cash controls.
  • Responsible for monitoring and reporting impact program has to hospital HEDIS measures
  • Report directly to KPH leadership on the progress of the program outlining successes and concerns, and corrective action plans to resolve concerns in specific time frames.
  • Participates as an active member of the Interdisciplinary Team.
  • Maintains compliance with and assists with ensuring facility compliance with all applicable State/Federal regulations and agency policy and procedure.
  • Maintain confidentiality regarding all patient information.
  • Complies with all agency policies and procedures.
  • Travel required between facilities.
  • Other duties as required by Administration

 

Qualifications

Education:

  • Licensed Masters Social Work required or
  • Registered Professional Nurse required.
    • RN must be licensed in New York State required.

 Experience:

  • Minimum of 5 years’ experience working in a hospital environment in the discharge planning or care coordination arena

Special Conditions of Employment:

  • Any and all additional eligibility requirements based on the specific position
  • Drug Test
  • Initial and continuous exclusion and sanction/disciplinary monitoring

Compensation:

 

$72,000-$80,000 Annual

 

The final offer will be determined after careful consideration of multiple factors such as relevant skills, years of experience and education.
